Clinical Flow People-
I recently evaluated a pleural fluid that had 70% B-cells. The
B-cells were negative for surface light chains by three sets of anti-light
chains-Tago FITC kappa and lambda paired with PE-CD20 (Leu16), Kallstedt
FITC kappa and lambda paired with PE-CD19 (HD37) and BD-monoclonal FITC
Kappa and PE-lambda. They were not plasma cells. They were + CD19, Cd20,
CD22, CD23 and FMC7 and half were + CD5. Cytologically the cells were
normal. The patient has no history or clinical suspicion of lymphoma.
Patient has a history of breast cancer. PCR analysis of frozen cells for
heavy chain rearrangements was polyclonal in pattern. Cytology swears that
the specimen was fresh and didn't sit around for a few days. What is your
favorite explanation for this? We've never had this happen before.
